CNYCC Announces Partnership with Unite Us

Central New York Care Collaborative Partners with Unite Us to Build a Coordinated Care Network Connecting Health and Social Care Providers Across the Region

Syracuse, New York -- Central New York Care Collaborative (CNYCC), today announced a new partnership with Unite Us to connect an extensive network of health, community-based and social service providers through a coordinated care system powered by the Unite Us technology platform. The CNYCC and Unite Us partnership will enable network partners across the region to send, receive and track electronic referrals in a transparent, accountable ecosystem that matches people with the providers who best meet their unique needs, ultimately to improve health outcomes across the region.

“This partnership will allow us to connect services in a meaningful way across the entire community”, says Cathy Homkey, Executive Director for the Central New York Care Collaborative. “Our network partners provide services that are vital to the health of our region and the technology of the Unite Us system will ensure those services are coordinated effectively.”

More than 100 organizations–including primary care, behavioral health, nursing home, specialty care, community-based organizations, and 11 local hospitals–have committed to join the Unite Us network and are excited, eager and ready to participate.  Each organization will be trained, onboarded and supported by Unite Us’ local, expert team members and will benefit both from the technology and data access to communicate seamlessly on care delivery including social care needs and services.

The partnership is part of CNYCC’s progression towards a fully integrated care delivery system. By including critical community-based organizations and social service providers, CNYCC signifies its understanding that whole-person care happens beyond the four walls of any single facility.
