CNYCC to distribute an additional $10 Million Dollars to Partner Organizations

A message from the Executive Director

Dear Valued Partner, 

I am pleased to share a Board approved Bridge Payment that will distribute an additional $10 million to CNYCC partner organizations over the next few weeks.  The “Bridge” Payment process will be available to contracted partner organizations for participation in the following areas: Above Target Actively Engaged Patient Reporting and New Performance Activities.  Below you will find an outline that provides details on each of these funding opportunities.

Bridge Payment Above Target Patient Reporting

CNYCC will be distributing approximately $1.5 million to partner organizations for actively engaged patient reporting above our PPS targets over DY1 and DY2.

As part of quarterly reporting to the NYS Department of Health, CNYCC was required to meet a minimum number of actively engaged patients across several projects.  When the minimum number of actively engaged patients for a project had been achieved, no additional funding was provided to partner organizations that reported patients above the target.  As part of the Bridge Payment Funding, CNYCC will retroactively provide payment to partner organizations that reported actively engaged patients over the identified targets.

The distribution of payment to eligible partner organizations for above target patient reporting is scheduled for the latter part of May 2017.

New Bridge Performance Activity Funding

CNYCC contracted partner organizations will also have an opportunity to receive funding through a process called the Performance Activity Fund.  Approximately $8.5 million will be distributed to partner organizations as part of the Bridge Performance Activity process.

All CNYCC contracted partner organizations will be provided the opportunity to complete as many as seven performance activities to receive funding.

A small cohort of contracted partners (approximately 40 partner organizations) will have the opportunity to participate in an additional activity focused on the implementation of the Population Health Management (PHM) system.  This cohort has been determined by partner contribution of impactful data to optimize the PHM system value for the network.

Please refer to the Bridge Payment Activities Checklist for a full listing of New Performance Activities (please note that a subset of the New Bridge Performance Activities list are activities that partner organizations may have already  completed).  The deadline for completion to participate in the Performance Activity process is Monday, June 19, 2017.

CNYCC Friday Webinar

A detailed overview of the Bridge Payment process will take place this Friday, May 19, 2017 during CNYCC’s Friday Webinar.  CNYCC's Director of Program Operations & Strategy, Lauren Wetterhahn, along with COPE Health Solutions will review the Bridge Payment process with a focus on the funding available through the completion of Bridge Performance Activities.

Please note that partner organizations will receive a time-sensitive Letter Agreement for DSRIP Activities Associated with the Bridge Funding Eligibility that must be signed and submitted prior to distribution of Bridge Payment funding.

New Payment Policies 

Finally, over the past few months, CNYCC leadership has been working closely with a Board approved Funds Flow Work Group comprised of representatives from several partner organizations across the PPS.  The work group has provided critical input in the development of the new payment policies.  The new proposed payment policies will distribute approximately $34 million in payments to contracted partner organizations and will place a greater emphasis on activities related to performance measures and preparation for value-based payment.

The new payment policies are currently being finalized and are anticipated to be presented for Board approval at the June Board of Directors meeting.  Upon Board approval, CNYCC will announce a formal process to amend existing project agreements (CNYCC Phase II Project Agreements) with contracted partner organizations.  The expected kick-off of the new policies is scheduled for early-July 2017.

Conclusion

In closing, I would like to thank you personally for your efforts.  As you can see, this is a very exciting time and we are working very hard to provide the necessary resources for our PPS to successfully transform to a new healthcare delivery model for the Medicaid population across our region.
