The Central New York Care Collaborative (CNYCC) recently announced a partnership with IBM Watson Health to create a Population Health Management (PHM) platform designed to help improve health for residents across the region. The platform will integrate IBM Watson Care Manager with broader Watson Health offerings and run on the Watson Health Cloud.
“By collaborating with IBM Watson Health, CNYCC will be able to integrate data from a variety of care settings — primary care, post-acute, behavioral health, community and acute care – including more than 75 electronic health record systems, health information exchanges, and other data sources”, said Virginia Opipare, Executive Director and CEO at CNYCC.
“The ability of our providers to access critical healthcare information through a population health management platform is critical to our success as we begin to prepare providers for the transition to a value-based care delivery model.”
CNYCC’s PHM system will aggregate the diverse data points and produce holistic patient insights incorporating clinical history, social determinants, and behavioral health. Clinicians will have the ability to access comprehensive information about their individual patients from the platform, utilize analytics to determine potential risk factors, and coordinate care for high-risk patients.The system brings together the cognitive computing power of Watson with IBM’s established expertise in population health management and social program management. Watson Care Manager combines care management best practices and automated workflows enabling care managers to create individualized care plans that address all determinants of health and engage individuals for better health.

Friday March 3, 2017 (10:00 - 11:00am) -- Join CNY Care Collaborative's Joe Reilly (Chief Information Officer) for an in-depth presentation and discussion of the Population Health Management (PHM) System Implementation Plan.
Friday March 10, 2017 (9:30 - 11:30am) -- Join CNYCC for a virtual demonstration of the IBM Watson Health Platform, and the elements that will be utilized in CNYCC's Population Health Management effort. Products to be demonstrated include: IBM Watson Care Manager, Explorys EPM Application Suite, Phytel Transition and Phytel Engage.
