Central New York Health Home Network, Inc. and SUNY Polytechnic Institute have partnered together to offer a certification course in Care Coordination Fundamentals.
When: Tuesday May 31, 2016 - July 21, 2016
Where: SUNY Poly campus.
Time: Classes meet Tuesday and Thursday evenings from 6:00-8:00 pm.
Cost: $300.00 per student
This 24 Module Curriculum will introduce the concepts of patient navigation and care coordination and help participants develop the knowledge and practical skills needed to provide these services. Students will experience a highly interactive class that will strengthen critical thinking skills through engagement in class discussions, individual exercises and group activities. Topics covered include care coordination basic skills, overviews of common chronic diseases and mental illness, health disparities, current statewide initiatives in healthcare, accessing patient resources and assisting with care transitions. Students will enhance their communication skill set to engage their clientele by learning key techniques of motivational interviewing, aspects of cultural humility and trauma informed care while mastering how to maintain professional boundaries and conduct successful home visits. Upon certification, students will be competitive candidates for positions in care management/care coordination; an emerging workforce that is growing rapidly in the midst of New York State healthcare reform.
Students must possess a minimum of a Bachelor’s Degree to take this course.
This is a non-credit bearing course, so no financial aid will be available.
All necessary materials are covered within the cost of the course and will be provided to course participants.
