One way that the New York State Delivery System Reform Incentive Payment (DSRIP) Program aims to accomplish its goals of healthcare delivery transformation is via the implementation of various projects. This includes cardiovascular. The goal of the cardiovascular project is to improve the management of cardiovascular disease and/or its associated risk factors using evidence-based best practices and strategies for disease management in the community and ambulatory care settings.
This project will include several efforts to address blood pressure control, cholesterol management and tobacco cessation, as well as stroke and cardiovascular disease prevention efforts including the use of aspirin where deemed appropriate. Other strategies focus on improving practitioner population management, implementing evidence-based clinical treatment guidelines, and reinforcement of blood pressure measurement using correct technique and equipment. Patients will be encouraged and supported by their care teams in the self-management of their chronic conditions.
